Code of the City of Royal Oak Chapter 720-15 

License expires one year from the date of approval. 

You will be asked to upload

• a copy of your driver's license
• a certificate of liability insurance 
• a plan of operation prior to submitting this application.

For each operator we need:

• Name
• Age
• Contact phone number
• Chauffeurs license number

Please fill out a box for each quadricycle with:

• Make
• Model
• Serial number
• Quadricycle license number

Once you submit your application, it will be reviewed to be sure that all information fields are complete.

Once the review is complete, you will be emailed a payment link for the $125 non-refundable application fee.  

Upon receipt of payment, your application will be sent for review and approval.  

You will be notified upon approval or denial.
